Studies defining the place for intervention have been difficult to perform and inconclusive. However, it is becoming clear that intervention makes only a modest contribution to blood pressure control. Furthermore, although ARAS is often present in elderly patients with renal impairment, the contribution of intervention to preventing progression of renal failure has been disappointing. The increasingly widespread use of angiotensin converting enzyme (ACE) inhibitors and angiotensin II receptor blockers (ARBs) has increased the clinical relevance of ARAS, and may be altering the indications for intervention. ARAS is more likely to end in cardiovascular death than renal failure, suggesting that it may be more rewarding to focus on the heart than the kidneys in this condition.

Role of renin-angiotensin system blockade in atherosclerotic renal artery stenosis and renovascular hypertension.

Current management of atherosclerotic renal artery stenosis (ARAS) remains controversial. A major issue is the condition’s functional significance; specifically, what is the contribution of ARAS to a specified patient’s heart failure, hypertension, or progressive renal disease? Clinicians will often consider whether revascularization of ARAS is likely to alter a patient’s symptoms or prognosis....

Angiotensin-converting enzyme inhibitor usage in patients with incidental atherosclerotic renal artery stenosis.

The efficacy of angiotensin converting enzyme (ACE) inhibitors in treatment of renovascular disease has been controversial. It has been reported that patients with incidental atherosclerotic renal artery stenosis (ARAS) are sometimes treated with ACE inhibitors before being considered for renal revascularization. This study was designed to describe the frequency and the characteristics of patie...

Inhibition of the renin–angiotensin system

• In patients with established evidence-based indications for inhibition of the renin–angiotensin system, for example congestive cardiac failure or chronic kidney disease with proteinuria, this treatment should not generally be withheld on the basis of renovascular disease. • When an angiotensin-converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB) are used in patients with re...

Introduction: Renal artery stenosis is the most prevalent disease of renal artery and has an important role in making hypertension and renal atrophy. Since in previous researches, despite high frequency of risk of cardiovascular disease in Iran, there did not exist any general research investigating risk factors of coronary artery disease and incidence of contemporary renal artery stenisis, ...

Analysis of Renal Artery Stenosis in Patients with Heart Failure: A RASHEF Study

BACKGROUND Previous data are controversial about the association of renal artery stenosis (RAS) with clinical outcome in patients with heart failure. Definition of RAS in previous studies might not be appropriate. By definition of RAS with renal duplex sonography, we investigated the association of RAS with clinical outcome in patients with heart failure. METHODS In this retrospective study, ...
